My ambition and focus of my research lies in being aware of the need to evaluate medicine as a service. For example, the lack of cooperation in hospitals and communities, through regional clinical experience such as visiting medical care and working at nursing care facilities. After that, as my doctoral dissertation, I could show the importance of a visiting doctor and the effect of housing renovations (Nikko Hygiene Journal 1990). At the Harvard Public Health Graduate School I came across the concept of health service research (hereinafter HSR)

We have consistently pioneered (1) the research of the nursing care insurance system, (2) parenting care physician, (3) actual condition of nursing care family, (4) evaluation of home/institutional care, and (5) public health by doctors, including opening the HSR field.

My main goal is to emphasize the importance of the family as a service-use regulating factor (IJQHC), the necessity of door-to-door transfer for regional rehabilitation (IJQHC, BMC Geriatrics), the relationship between use of short-stay and the caregiving degree of the individual (BMC Geriatrics), and furthermore in a special edition of The Lancet on Japan, I introduced nursing care insurance to the world, it showed that it is effective only for high-income class caregivers by using nationwide data analysis (Lancet). In addition, the greatest cause of nursing care insurance expenditure was shown to be high numbers of caregiver at care facilities (BMC HSR), a suppression factor for the use of visiting nursing (BMC Geriatrics) etc. ② The promoting factors of home general clinics are shown jointly with the medical association (Journal of Primary Care Association), and from the analysis of opinions by doctors, research showing the importance of family doctor, international comparison of primary care doctors. ③ We studied the relatively shorter survival time of those being nurse by their wife (BMC Geriatrics), etc. ④ the effectiveness of visiting nursing (GGI) for continuing home life, the doctor's role (Health Policy) in facility care, Outcome assessment (GGI), etc., ⑤ Basic data for lonely death measures as shown by forensic data (BMJ open).

With my colleagues in the laboratory, I would like to contribute to creating a mechanism that can deliver high-quality services to the people in need through health services research with the motto "medical treatment in harmony with life".
